Specimen Requirements
ThinPrep vial without collection device (approx. 20 mL), containing exfoliated cells from the cervix or vagina.
Collection instructions:
1. Label PreservCyt® vial with the patient’s name and date of birth or requisition label.
2. Include all pertinent clinical information (LMP, hormone use, prior Pap smear and biopsy results) on the test requisition.
3. If lubricant is essential to insert the speculum, use a minimal amount of a water based lubricant and avoid those withcarbopolymers (carbomers).Prior to specimen collection,gently dabaway any visible blood, mucus and/or discharge from the cervix.
4. For collection with a broom like device; (a) insert the central bristles of the broom into the endocervical canal deep enough to allow the shorter bristles to fully contact the ectocervix. Push gently, and rotate the broom in a clockwise direction five times; (b) rinse the broom as quickly as possible into the PreservCyt® solution vial by pushing the broom into the bottom of the vial 10 times, forcing the bristles apart. Swirl the broom vigorously to further release material. Discard the collection device; do not leave in Pap vial. Tighten the cap so that the torque line on the cap passes the torque line on the vial.
5. For collection with endocervical brush/spatula; (a) obtain adequate sampling from the ectocervix using the plastic spatula; (b) rinse the spatula as quickly as possible into the PreservCyt® solution vial by swirling the spatula vigorously in the vial 10 times. Discard the spatula. (c) Obtain an adequate sampling from the ectocervix using an endocervical brush device. Insert the brush into the cervix until only the bottom-most fibers are exposed. Slowly rotate 1/4 or 1/2 turn in one direction. Do not over-rotate. (d) Rinse the brush as quickly as possible into the PreservCyt® solution vial by rotating the device in the solution 10 times while pushing against the PreservCyt® vial wall. Swirl the brush vigorously to further release material. Discard the brush; do not leave in Pap vial. (e) Tighten the cap so that the torque line on the cap passes the torque line on the vial. For best results, specimens should be taken at mid-cycle (between day 12 and 18) although this is not essential. Specimens should never be taken during active menstruation.
· Patients should be instructed to refrain from intercourse, douching and the use of intravaginal medications for 48 hours prior to examination.
Clinical Significance
The ThinPrep® Pap test is a screening test used in the detection of cervical cancer, pre-cancerous lesions, infection and other cytologic categories as outlined by the Bethesda system for reporting cervical cytology.

Comments
High-Risk Human Papillomavirus, Chlamydia trachomatis & Neisseria gonorrhea, and Trichomonas molecular testing may be run out of the ThinPrep Vial concurrently with Pap. Vial eligible for molecular testing for up to 6 weeks from collection.
