Specimen Requirements

Bone marrow aspirate, 3mL preferred, 1 mL minimum, in a dedicated sodium heparin or EDTA tube for FISH assay. Sample should be stored at room temperature and transported to the laboratory within 24-48 hours upon marrow collection. Tissue in saline, RPMI or fresh can be tested using direct harvest instead of enrichment, provided the plasma cell percentage is at least 15%.

Diagnostic Utility

This test is designed for diagnostic specimens from patients with multiple myeloma or other plasma cell proliferative disorders in support of diagnosis of new cases and identifying prognostic markers to guide the therapies. Probes sets of 1p/1q, TP53, 13q, 9/15, IGH with reflex to CCND1(MYEOV)/IGH, FGFR3/IGH, and IGH/MAF are included in this FISH panel. Presence one of the cytogenetic abnormalities is supportive for the diagnosis of multiple myeloma or other plasma cell neoplasms. However, the FISH results should be interpreted in context with other clinical, morphological, and FLOW findings.

Cause for Rejection

Serum or plasma. Frozen specimens. Clotted marrow aspirates. Severely hemolyzed specimens. Specimen containers without two (2) patient identifiers.

Retention

Fixed CD138 enriched cells stored frozen for 7 days after reports being signed out.

Comments

If enriched samples are insufficient to complete the whole panel, probe sets of TP53, MYEOV, 1p/1q, and/or 9/15 will be prioritized unless specific instruction received from the referral clinics.
