Specimen Requirements

Formalin-fixed, paraffin embedded tissue block or cell block, or 1 unstained charged slide for each test requested plus 3-4 additional unstained slides cut at 4 microns. Adequate space on each slide for control tissue is required.

Diagnostic Utility

The chromogenic in situ hybridization (CISH) uses a probe with sequence complimentary to a segment of kappa light chain mRNA. It is useful to identify kappa light chain mRNA in the cytoplasm of plasma cells and plasmacytoid lymphocytes, which in turn may help determining kappa or lambda light chain restriction, a surrogate for clonality. CISH has the advantage of avoiding background staining from serum frequently seen with antibody in immunohistochemistry.
